You have to have a family income within certain income ranges to qualify for Obamacare. Texas did not participate in the federally-funded Medicaid expansion, so poor people who fall in that gap do not qualify. People who are too wealthy for Obamacare subsidies sometimes don't want to buy health insurance, also.

It would be meatloaf if there was some kind of filler, like bread, crackers, or oats. Meatloaf was developed to stretch a pound of ground beef out to feed a big family. This recipe is just super-spiced ground beef.
