YouTube user BrianRimmer compiled every opening theme for Doctor Who in chronological order. At the video link, you can get the details.

I watched a little bit of the Fourth Doctor in the early 90s, but found the show very difficult to understand. Certainly after 43 years on air, it’d be hard to gain a familiarity with the series.

If a person who has never seen Doctor Who were to watch only one episode, which should it be?

    I believe you meant the early 70s as opposed to the early 90s if you began watching during the fourth Doctor (or did you begin watching tape/reruns?).

    Well, it was from the 70s, but I watched those episodes in the early 90s. I watched reruns on PBS.

    Agree with Vonskippy. Blink is probably the best standalone Doctor Who episode.

    However, the newest season with Matt Smith is actually a fantastic place to start since it heralded a new Doctor, a new companion, a new head writer, and new producers. I’d recommend starting there if you’re at all interested.

    I also recommend Blink, but it’s because you learn about the Doctor as Sally does, so there’s no need to know anything about the show before watching it.
