Pretty sure it's a tapeworm trap. Tapeworms can't resist slaloming through such things (they're born slalomers) and once they're halfway through, you extract the trap, tie the ends together and dispose of properly. The hook and chain have to do with extraction but I intentionally glossed over that part.

If it weren't for complaints, we'd get no comments at all. Think about that when you want to encourage this or that type of post on this site.

While I agree that this site is probably not the place for overtly political statements, I have a hard time seeing this as such a statement. While it was attached to someone political, I'm fairly certain that the grand majority of people, despite their political beliefs, understand the original statement was ridiculous at best.
While I also didn't particularly enjoy all pregnancy being likened to drowning, you have to keep in mind it's satire - that's the point. If you don't get that, you probably shouldn't read A Modest Proposal.
